In an effort to keep everyone safe and minimize paperwork we rely on you to complete online paperwork prior to your appointment. We use a platform called WaiverKing to allow you to virtually complete all necessary documents for your upcoming visit. We appreciate your effort in helping us provide you the best possible care and focus to help you achieve your treatment and training goals.
